Multidisciplinary Rounds on DRH 63
- Rounds occur every weekday at 0930 on unit 63 at the nurse’s station or on Zoom
- Participants include: Providers for general, vascular, and bariatric surgery, 63 charge nurse, case manager, pharmacist, utilization manager, and others
- Goal is to run through each surgical services list of patients that are on unit 63, allow the team to discuss each patient, ask questions and solve care problems, especially related to discharge.
- Run through the 27 patients in 30 minutes to 45 minutes
Brief History
- Provider: Start with the room number and name of the patient and then give a 30 second history of why they are here, what they had done.
Plan While admitted
- Provider: next describe the plan, and what we are doing for the patient. Field any questions from the team about the plan.
- New for 2023 also discuss VTE prophylaxis status
Discharge Planning
- Team:
- Dispo planning, discharge location home, SNF, LATC etc.
- Are there any anticipated needs at discharge such as patient education, home health, home physical therapy etc.
- Are any medications an issue for the patient?
- Are there any barriers to discharge?
- Expected day of discharge (EDD)
- Transportation Issues?
